Purpose: Evidence based practice is an integral part of Public Health Nursing. PHNs are expected to identify health problems in the population and address them using a systematic approach; the PHN practice model and 17 interventions. This assignment gives each student the opportunity to identify a health problem (communicable or chronic disease), address the identified problem utilizing appropriate interventions, while addressing one of the Healthy People 2020 objectives.

Description: Each student will choose a communicable or chronic disease and a related Healthy People 2020 objective. The student will write a 5 page paper, in APA format that includes the following components:

a. An overview of disease

b. Description of the risk/causal factors related to the disease

c. Description of the Healthy People 2020 objective and goal related to the disease or a risk/causal factor related to the disease

d. Description of the evidence-based interventions identified to prevent or mitigate the risk/causal factors related to the communicable or chronic disease.

(Utilizing an example of an intervention identified in a research article)

e. Description of the implementation of 3 PHN interventions used to mitigate the risk/causal factors.
